2021 HIGH HOLY DAY TICKET REQUEST FORM
Please note that all individuals over the age of twelve who plan to attend in person, must submit a Vaccination Registration Form, along with proof of vaccination. This form is included in this packet and will also be available online
Tickets, without charge, are provided for the following categories:
A) Members
B) Children beyond high school living at home, including college, who are dependents.
C) Children of Religious School age.
D) Permanent members of your household.
E) Your child and their spouse for the first year of their marriage.
F) If full Family Annual Support ($2,100.00) or Full Single Annual Support ($1,260.00) is paid in full prior to August 30, that member is entitled to 4 extra High Holy Day tickets for extended family and friends.
